What is the Modelo EX-01?
The Modelo EX-01 — Solicitud de autorización de residencia temporal no lucrativa — is the official Spanish government form for the non-lucrative residence authorisation that sits behind the Non-Lucrative Visa (NLV). It is issued under Organic Law 4/2000 and the immigration regulation approved by Royal Decree 1155/2024.
The form itself is short — three pages, including the instructions — but it is where the consulate records exactly who is applying, in what capacity, and where you can be notified. Your evidence of income or savings, health insurance, criminal record and medical certificate goes in your supporting documents, not on the form.
- Every applicant completes their own EX-01. A couple applying together submits two forms; each child has their own, signed by a parent as legal representative.
- Initial applications are filed at the Spanish consulate responsible for where you legally live. Renewals are filed electronically from Spain, using the same form.
- The form is free. It is published by the Ministry of Inclusion, Social Security and Migration and is marked “impreso gratuito — prohibida su venta”.
- Most consulates also ask for the national visa application form. Check your consulate’s own document list — it is the final word on what to bring.
Make sure you are using the current version
The current EX-01 cites RD 1155/2024 at the top of the first page. Older copies that cite RD 557/2011 are out of date: the regulation was replaced on 20 May 2025, and the current form has a different layout — notably a dedicated section for family members whose application relies on the main applicant’s financial means. Download the form from the Ministry’s official forms page or from your consulate’s website, rather than reusing an old copy.
How to complete the EX-01, section by section
Section 1 — The applicant (Datos de la persona extranjera solicitante)
- Passport number exactly as printed. Leave the N.I.E. boxes empty unless you already have a Spanish foreigner’s identity number.
- Surnames and first name exactly as in your passport, including second surnames and middle names. No nicknames or initials.
- Sex, date of birth (dd/mm/yyyy), place and country of birth, nationality and marital status (S single, C married, V widowed, D divorced, Sp separated).
- Father’s and mother’s first names.
- Address in Spain if you already have one, plus a mobile number and email address you check regularly.
- Legal representative — only for minors or people who act through a representative.
- School-age children in Spain — tick yes or no.
Section 2 — The family member who holds the financial means (if applicable)
Complete this section only if you are a family member applying on the strength of the main applicant’s income or savings — typically a spouse, partner or child. Enter the main applicant’s passport details, name, date and country of birth, parents’ names and your relationship to them (parentesco). The main applicant leaves this section blank on their own form.
Section 3 — Representative presenting the application (if applicable)
Only needed if another person or organisation formally presents the application on your behalf. If you are submitting it yourself at the consulate, leave it empty.
Section 4 — Address for notifications
The person and address that should receive official notifications. The box consenting to electronic notification through the government’s DEHú system requires a valid digital certificate or Cl@ve access; most first-time applicants applying from abroad do not have one yet. Filling in the mobile and email fields lets the authorities send an informative alert when a notification is waiting.
Section 5 — Type of authorisation requested
- For a first application, tick Residencia inicial.
- Then tick Titular de los recursos económicos if you are the main applicant whose means support the application — or Familiar del titular de los recursos económicos if you are a family member relying on those means.
- At renewal, the same form is used with Renovación ticked instead.
Date, signature and the “Dirigida a” line
Add the place and date and sign the form — the applicant signs, or the parent or legal representative for a minor. The “Dirigida a … Código DIR3 … Provincia” line identifies the office the application is addressed to; follow your consulate’s instructions for it.
Rules from the form’s own instructions
- Complete it in capital letters, in black pen and block print — or type it.
- Write dates as dd/mm/yyyy.
- Submit the original and a copy of the form.
Common mistakes to avoid
- Using an old version that cites RD 557/2011.
- Names or passport numbers that don’t match the passport exactly.
- One form for the whole family instead of one per person.
- The main applicant filling in Section 2, or a family member leaving it empty.
- Ticking “Titular” on a family member’s form — family members tick “Familiar del titular”.
- Leaving Section 5 blank or unsigned forms — check every page before your appointment.
- Expecting the form to show your income: your financial means are proved with documents, and they must cover the whole household (for 2026, €2,400 a month for the main applicant plus €600 for each family member).
