Health Insurance Requirement 2026
Health Insurance Requirement for Spain's Non-Lucrative Visa
Health insurance for the non lucrative visa has to meet specific rules, and the wrong policy — NHS entitlement, travel cover, or a policy with copayments — can stop an application at the consulate. Here is what your policy and certificate need to show.
Health insurance on its own
Already arranging your visa yourself or through another company?
You can arrange your health insurance through us separately. You don’t need to use our visa service.
Whether you’re applying independently, working with another visa company or adviser, already hold your visa and need ongoing cover, or simply want private health insurance without a visa — you’re welcome to request a quote. You don’t need to buy our visa service or create a visa-dashboard account to request an insurance quote.

The Requirement
What Must Health Insurance for Non Lucrative Visa Spain Include?
Spanish consulates check each of these points. A policy that meets them all — with a certificate that says so — is what gets your insurance accepted first time.
From an authorised Spanish provider
The insurer must be authorised (registered and regulated) to provide health insurance in Spain — registered with Spain's Dirección General de Seguros y Fondos de Pensiones (DGSFP). Not all international health insurers meet this criterion. The policies we quote come from an insurer authorised to operate in Spain.
No co-payments (copagos)
Co-payments — where you pay a percentage or fixed fee each time you visit a doctor, use a service, or fill a prescription — are explicitly not accepted. Your policy must provide full coverage with no out-of-pocket costs per appointment or treatment. This eliminates many standard health insurance products that include co-payment structures.
No excess or deductibles
Policies with an excess (an amount you must pay before insurance covers you) are not accepted. Full comprehensive cover must apply from the first euro of any medical expense. This is a common point of failure for people who purchase standard international health insurance not specifically designed for the NLV.
Full duration coverage — minimum 1 year
Your policy must cover the full duration of your visa — a minimum of one year. Travel insurance or short-term health policies with expiry dates before the end of your visa period are not accepted. Annual renewable policies that clearly cover the visa period are the standard for NLV applications.
Valid across all of Spain
The policy must provide coverage throughout all of Spain — not just in a specific region or city. Some cheaper plans have geographic restrictions; these are not acceptable. Whether you plan to live in Madrid, Barcelona, the Canary Islands, or rural Andalucía, your policy must be valid wherever you are.
Comprehensive medical cover
The policy must cover full medical care equivalent to Spain's public health system — primary care (GP visits), specialist consultations, hospitalisation, emergency treatment, surgery, and prescription medication. See the document checklist for what your insurance certificate must state.
Not Accepted
What the Consulate Will NOT Accept as Health Insurance for Spain's NLV
These are the most common health insurance mistakes — each one will result in your application being refused or your appointment being turned away.
NHS entitlement
UK applicants sometimes assume their NHS entitlement covers them abroad or can be used as evidence for the NLV. It cannot. Once you move to Spain as a resident, you are no longer entitled to NHS care. The consulate explicitly requires private insurance from a Spanish-authorised provider.
EHIC / GHIC cards
The European Health Insurance Card (EHIC) and the UK Global Health Insurance Card (GHIC) are for temporary stays as visitors, not for residents. Neither is accepted for the NLV under any circumstances.
Travel insurance
Standard travel insurance — including annual multi-trip policies — is not accepted. Travel insurance is designed for short trips, typically has low coverage limits, and is not from a provider authorised to operate in Spain as a long-term health insurer.
Policies with co-payments
Any policy that charges you a fee per GP visit, specialist appointment, prescription, or hospital admission is not acceptable. Even small co-payments of €5–€10 per visit make a policy non-compliant. Always check the policy schedule carefully.
Policies with excess or deductibles
Any policy with an excess — an amount you must pay before insurance kicks in — is not acceptable. If the policy documents mention an excess, deductible, or self-insured retention of any amount, the policy is non-compliant regardless of how small the amount is.
Providers not authorised in Spain
Some international health insurers — even large, reputable ones — are not specifically authorised to provide health insurance in Spain. If the provider is not registered with Spain's DGSFP, the policy will not be accepted regardless of its other terms.
The Certificate
The Insurance Certificate the Consulate Checks
Consulates look at your insurance certificate, not your policy booklet. Make sure it shows:
Who is covered
Your full name as in your passport, the policy number and the insurer. On a family application, each person needs to be named.
What is covered
Full medical cover throughout Spain, with no copayments for covered services.
When cover runs
The start date and the period covered. Many consulates also ask for proof that the first year is paid, so visa policies are normally paid annually.
The certificate is issued once your policy is accepted and paid. See what the NLV insurance certificate includes →
Timing
Start Date and Payment
Have your certificate ready for submission. Agree the policy start date against your intended arrival and your consulate’s current requirements. No copayments for covered services. Benefits, exclusions and authorisation requirements are set out in the policy.
If you use our managed NLV service, your case manager will suggest the right start month for your appointment.

Common Questions
Health Insurance for Non Lucrative Visa Spain — FAQ
What health insurance do I need for Spain's Non-Lucrative Visa?
Health insurance for non lucrative visa Spain must be: from a provider authorised to operate in Spain; comprehensive with no co-payments (copagos); no excess or deductibles; valid for the full duration of the visa (minimum 1 year); and valid across all of Spain. NHS entitlement, EHIC/GHIC cards, and standard travel insurance are all explicitly not accepted. The wrong policy can stop an application at the consulate.
Does the NLV health insurance policy need to have no co-payments?
Yes — this is a hard requirement. Co-payments (copagos) — where you pay a fixed fee or percentage per GP visit, specialist appointment, prescription, or hospital admission — make a policy non-compliant. Even small co-payments of €5–€10 per visit are not acceptable. Your policy must provide full comprehensive cover with no out-of-pocket costs per treatment. This eliminates many standard health insurance products.
Can I use my NHS entitlement or EHIC/GHIC card for Spain's NLV?
No. NHS entitlement, EHIC cards, and GHIC cards are all explicitly not accepted for Spain's Non-Lucrative Visa. You must hold private health insurance from a provider specifically authorised to operate in Spain. This is a hard requirement with no exceptions — there is no workaround.
How much does health insurance for Spain's NLV cost?
The price depends on your age, the cover you choose and how many people are insured. Request a personalised quote through spanish-healthinsurance.com — we normally reply within 24 hours, often sooner.
When should I buy my health insurance for the NLV?
Have your certificate ready for submission. Agree the policy start date against your intended arrival and your consulate’s current requirements.
Do all family members need separate NLV health insurance?
Yes — every family member on the NLV application needs compliant cover and must be named on the certificate. We can quote the whole household together through spanish-healthinsurance.com.
What if my health insurance policy expires while I am in Spain on the NLV?
You must maintain compliant private health insurance for the entire duration of your time in Spain on the NLV. To renew your residence authorisation, you need to show that you kept health insurance throughout your current permit and that it continues. Most NLV holders simply keep their annual policy running. See health insurance for your NLV renewal.
I have a pre-existing condition — can I still get NLV-compliant health insurance?
Declare your medical history fully. The insurer confirms whether it can offer cover and on what terms. Any exclusion may affect suitability for your visa, so confirm this before relying on the policy.
Does NLV health insurance need to include repatriation?
Some consulates and policies treat repatriation differently. Check your consulate’s current checklist, and your quote will confirm whether the policy includes it.
How can I check a policy before I buy it?
Check that the insurer is authorised in Spain (registered with the DGSFP); that there are no copayments and no excess; that cover is full medical cover throughout Spain; that it runs for the whole visa period; and that the certificate names everyone on the application, as in their passports.
Do consulate requirements vary?
The core requirements are the same, but each consulate publishes its own document list and some ask for extras such as proof that the first year is paid. Check your consulate’s current list before your appointment.
