Best Health Insurance for the Spain Non-Lucrative Visa
The best health insurance for Spain's non-lucrative visa is a policy the consulate accepts first time — full cover in Spain, no copayments, no excess, from an insurer authorised in Spain. Here is what to look for, and how to get your quote whoever handles your visa.
NLV Health Insurance Requirements
These are the points consulates check. A policy that misses one is likely to be questioned at your appointment.
-
✅
Comprehensive coverage in Spain — must cover general practitioner, specialist consultations, hospitalisations, emergency treatment, diagnostic tests, and surgery.
-
✅
No co-payments (copagos) — you must not be required to pay any portion of the cost when you access healthcare. Even a small co-payment makes the policy non-compliant.
-
✅
No excess or deductible — you must not have to pay a set amount before the insurance covers your costs. Any excess, however small, is a disqualifying feature.
-
✅
Provider authorised to operate in Spain — the insurance company must be registered with Spain's Directorate-General of Insurance (DGSFP) and authorised to operate in Spain. Many international insurers are not.
-
✅
Covers the full duration of the visa — the policy must be valid for at least the full duration of the visa being applied for (typically one year for the initial NLV). A policy that expires before the visa does is insufficient.
-
❌
Travel insurance does not qualify — however comprehensive it may be. Travel insurance is not the same as private health insurance and will be rejected by the consulate without exception.
Choosing Suitable Cover
Once a policy meets the consulate’s requirements, these are the points that make it right for you. Your quote sets out the exact policy terms.
Cover throughout Spain
A policy for the NLV needs to cover you throughout Spain with no copayments for covered services, and come with a certificate for your file.
Cover when you travel
If you’ll spend time outside Spain, ask about cover abroad. It varies between policies, and your quote sets out what applies.
Your age and health
Policies have age limits for taking out cover, and each person answers a health declaration. We check eligibility before you apply.
Everyone named
On a family application, each person is insured and named on the certificate, so the household is quoted together.
Already arranging your visa yourself or through another company? You can buy your health insurance through us whoever handles your visa — no visa-service purchase or dashboard account needed. Your insurance quote will be handled by Spanish Health Insurance.
Using our managed NLV service? Your case manager checks your insurance certificate before your dossier is finalised — whichever policy you hold.
What to Look For in an NLV Health Insurance Policy
Beyond the mandatory requirements, these are the features that matter most for your daily life in Spain.
No Co-payments (Copagos)
The absolute must-have. Covered consultations, tests and treatments must carry no copayment. A policy that requires you to contribute even €5 per visit fails the NLV requirement.
No Excess or Deductible
Equally non-negotiable. There must be no threshold amount that you pay before insurance kicks in. Zero excess, zero deductible — on every claim.
Spain-Wide Coverage
Your policy must provide comprehensive coverage across all of Spain — not just in specific regions or cities. Check that coverage extends to wherever you plan to live, including the Canary Islands, Balearics, Ceuta, and Melilla if relevant.
Provider Authorised in Spain
The insurer must be registered with and authorised by Spain's DGSFP (Dirección General de Seguros y Fondos de Pensiones). This is a regulatory requirement — not all well-known international insurers qualify.
English-Language Service
Many expats benefit significantly from being able to contact their insurer in English when navigating the Spanish healthcare system. Check that customer service and claims handling are available in English.
Family Plans Available
Every family member on your NLV application needs cover named on the certificate. Ask for a household quote so everyone is covered from the same start date.
How Much Does NLV Health Insurance Cost?
Your premium depends on:
- Age — premiums rise with age, and each plan has a maximum age for taking out cover
- The plan — including whether you want cover when you travel outside Spain
- Who is covered — each family member is insured and named on the certificate
- Payment — visa policies are normally paid annually, because consulates commonly ask for proof that the first year is paid

Common Health Insurance Mistakes That Cause NLV Rejection
These are the insurance mistakes we see most often — and how to avoid them:
- Buying travel insurance instead of health insurance. Travel insurance — even comprehensive annual travel insurance — does not meet the NLV health insurance requirement. The consulate will reject it. You need private health insurance, not travel insurance.
- Buying a policy with an excess. Many standard international health insurance policies include an excess (e.g., €500 per claim or per year). Any excess, however small, makes the policy non-compliant for the NLV. Read the policy terms carefully, or ask us for a policy arranged for visa applicants.
- Buying from a provider not authorised in Spain. Some well-known international insurers are not authorised to operate in Spain. An otherwise comprehensive policy from a non-authorised provider will be rejected. Check the insurer is authorised in Spain before you buy.
- Buying too early and letting the coverage expire. Agree the policy start date against your intended arrival and your consulate’s current requirements, and keep the policy running so there is never a gap in cover.
- Not covering all family members. Every family member on your NLV application requires their own health insurance certificate. Forgetting to insure children or a spouse is a surprisingly common oversight.
- Assuming your existing insurer is compliant. If you already have international health insurance, do not assume it meets NLV requirements. Check specifically: no co-payments, no excess, Spain authorisation. Many policies that are excellent for other purposes fail one or more of these criteria.
Frequently Asked Questions
What health insurance is accepted for the Spain NLV?
NLV-compliant health insurance must provide comprehensive coverage in Spain, with no co-payments, no excess, and from a provider authorised to operate in Spain. The policy must cover the full duration of the visa applied for. Travel insurance, policies with co-payments, policies with an excess, and policies from non-authorised providers are all rejected. Every policy we quote for the NLV comes with a certificate for your application.
Can I use the NHS (or my home country's health service) for the NLV?
No. The NLV requires private health insurance from a provider authorised to operate in Spain. Access to the NHS, Medicare, or any other home-country state health service does not satisfy this requirement. The requirement is specifically for private health insurance in Spain — because as an NLV holder you will not have access to Spain's public healthcare system (until you qualify for it through long-term residency or other routes).
How much does NLV health insurance cost per year?
It depends on your age, the cover you choose and how many people are insured. Request a personalised quote — we normally reply within 24 hours, often sooner.
When should I buy my NLV health insurance?
Have your certificate ready for submission. Agree the policy start date against your intended arrival and your consulate’s current requirements. Allow time for the policy to be accepted and paid so your certificate is ready.
What if I get sick before I move to Spain?
If you become ill or receive a new diagnosis before your health insurance policy starts, this may be treated as a pre-existing condition by the insurer, which could affect coverage or premiums at the time of renewal. If you become seriously ill before your consulate appointment, you may need medical advice on whether to proceed with your visa application. Our case managers can advise on the implications for your NLV application. It is important to disclose any pre-existing conditions honestly when purchasing your policy.
Does my whole family need health insurance?
Yes. Every family member included in your NLV application must have their own NLV-compliant health insurance. The consulate requires a certificate for each applicant — both the main applicant and all dependants including children. Ask for a household quote so everyone is covered from the same start date.
How do I choose the right NLV health insurance?
Start with what the consulate checks: cover throughout Spain, no copayments, no excess and an insurer authorised in Spain. Then decide whether you want cover when you travel outside Spain. Eligibility and terms are confirmed for each person when you apply, and we check before you rely on a policy.
Can I use my EHIC or GHIC card instead of private health insurance for the NLV?
No. The European Health Insurance Card (EHIC) or Global Health Insurance Card (GHIC) does not satisfy the NLV health insurance requirement. These cards provide access to state healthcare during temporary stays — they are not private health insurance and are not accepted by Spanish consulates for the NLV. You need a full private health insurance policy from a provider authorised in Spain, with no co-payments and no excess.
